Requesting an assessment
Complete the Social Work - Contact Us form with as much information about what you need assessed and why. You must give us your telephone number. If you're requesting on behalf of someone else, you need their permission unless you are legally authorised to act on their behalf.
We need as much information as possible:
- details of your home layout
- health conditions you have
- how your health affects your ability to do daily tasks
- what solutions you've already tried
Call us on 03451 55 15 03 if you prefer and our Social Work enquiries team will start the process with you. They may be able to give you some advice or assistance directly. If not, they will pass your request to the Community Occupational Therapy team.
Home visit
In many situations, we need to visit you at home to carry out a full assessment.
We have a high demand for assessments and operate a waiting list. This waiting list is prioritised based on our eligibility criteria. Priority is based on risk and how this can be reduced while you're waiting.
Costs
- Information and advice is free.
- Minor adaptations are free (through Fife Council or Housing Associations).
- Structural adaptations in privately owned properties may incur a 20% cost.
- Housing disability adaptation grants are available.
