Fife Health and Social Care Partnership Holistic Needs Assessment Privacy Notice

Levenmouth Test of Change - supporting people to live well with long term conditions.

When you contact Fife Health and Social Care Partnership (or a referral is made on your behalf with your verbal consent) to request an appointment to discuss “what is important to you”, a holistic needs assessment will be completed.

This assessment tool was developed by Macmillan Cancer Support and is being delivered by your care provider, Fife Council. The assessment tool facilitates a conversation between yourself and the key worker to explore the social, emotional and practical impacts that your long-term illness may have. This conversation results in a personalised care and support plan identifying what is important to you. The completed personalised care and support plan is shared with you and the referring agency.

and those agencies, such as Fife Carers Centre, Fife Forum etc. who have contracts with Fife Council or NHS Fife to provide care and support services.

The Health and Social Care Partnership will not keep your information for longer than is necessary. In some instances, the law sets the length of time information has to be kept. What information do we collect about you?

We collect personal information about you when you contact the service, one of our partners/agencies, or your health professional refers you to the service. It is important that the information recorded on your assessment form is accurate and you should go over it with the worker completing the form to make sure it is correct. We also collect information about you when you voluntarily complete evaluation questionnaires, participate in online consultations, provide feedback and contribute to case studies.

How will we use the information about you?

We collect and share information about you to manage your assessment and care plan, we may also signpost/refer you to other services.

We will also anonymise your data and use this information to evaluate this test of change project. We will provide anonymised information to Macmillan and NHS Fife for the purpose of evaluation and review.

You have the right to request a copy of the information that we hold about you. If you would like a copy of some or all, of your personal information, please email information.requests@fife.gov.uk.
