This form is for booking an in-person appointment with a Customer Service Advisor, who will assist you in applying for, or replacing a National Entitlement Card (NEC), previously known as a MyFife card.

Appointment length

• 15 minutes

What you'll need for the appointment:

• proof of identity (passport or driving licence)
• proof of address (dated in the last 3 months, such as Council Tax bill, utility bill, letter from school or NHS)
• proof of disability (award letter dated in the last 18 months)

If you are applying for your child, you will also need to provide:

• proof of identity for your child (full birth certificate, adoption certificate or passport)
• proof of parental relationship
