When completing the form you MUST:

- Send it to us by email at FHR.Team@fife.gov.uk as an email attachment
- Include your name, address and FHR number (if known) in the email subject heading
Please complete the Access to children/shared custody of children - signed confirmation form if you wish to confirm your overnight access/shared custody arrangements.
Complete the Housing functional needs assessment form if you, or someone who is to be rehoused with you, has a health condition and/or disability which is made worse your current housing situation.
Complete the Habitual residence test for returning UK nationals form if you are a UK National who has returned to live in the UK within the last two years.
Complete the Request for support assessment form if you need to live close to someone to give or receive support.
