Use this form to let us know about your child's special dietary requirements.

Your child may have specific dietary needs due to medical, cultural or religious reasons.

You will need to provide:

• the nursery, primary, or secondary school your child attends
• the class they're in
• the type of dietary requirement your child has, with details
  

Medical evidence must be provided if it's a medical dietary requirement. This could be:

• a letter from a clinic
• visit with a nutritionist or dietitian
• a letter from a GP issued at the time of diagnosis

We do not require a newly issued GP letter, as we understand this may take time and could incur a cost.

You will also be asked if you want to have a meeting with Catering Management. This allows us to discuss any possible menu changes.

If you do not want a meeting, we will not make any menu changes. You or your child will need to select suitable meals from the standard menu.

Note:

• a new form will need to be completed each year, as our menus are updated annually
• if you are unable to submit the form electronically, paper copies are available from your school office and can be completed and returned for scanning to the dietary team
